The Execution Attempt
Prison officials at Riverbend Maximum Security Institution in Nashville administered two doses of the chemical pentobarbital to the 50-year-old inmate. Witnesses and her legal team reported that her heart was still beating, she had not lost consciousness, and she could be heard breathing and snoring. According to the Associated Press, the witness room curtain was closed twice and media were eventually asked to leave. Prison officials then transported Pike to an off-site medical facility where she received life-saving measures, according to her lawyer.
Legal Battle and Background
The attempted execution followed a last-minute legal battle that ended when the US Supreme Court vacated a temporary stay issued by the Sixth US Circuit Court of Appeals. The appeals court had halted the process to review claims that Pike's history of severe childhood abuse and mental health issues, including bipolar disorder and post-traumatic stress disorder, were not presented during her original sentencing. Pike was 18 years old in January 1995 when she, her 17-year-old boyfriend Tadaryl Shipp, and Shadolla Peterson took 19-year-old Colleen Slemmer to a wooded area in Knoxville. The three were associates at a Job Corps vocational training center. Court records state they tortured Slemmer for 30 to 45 minutes using a box cutter, a small meat cleaver, and a rock, and carved a pentagram into her chest while she was alive. A jury convicted Pike of first-degree murder and conspiracy, sentencing her to death in 1996 as the only participant to receive a death sentence. Shipp received life in prison with the possibility of parole, while Peterson received a different sentence.
State Response
Tennessee Governor Bill Lee ordered a comprehensive third-party review of the incident and paused the state's final scheduled execution for the year. Department of Correction spokesperson Dorinda Carter stated that the agency followed all steps of the state-approved execution protocol. This incident follows an earlier event in May 2026 where the execution of inmate Tony Carruthers was postponed because staff could not find a vein for the lethal drug.
Context
Capital punishment protocols in US state prisons typically outline precise procedures for administering lethal substances, but rare complications during executions have occasionally prompted legal challenges and reviews. Tennessee uses pentobarbital for lethal injections and maintains specific guidelines overseen by the state department of correction. Legal appeals in death penalty cases frequently examine whether mitigating factors like youth or childhood trauma were adequately weighed during initial jury trials.
